Form What’s It Used For
Preflight Waiver Form to be used for preflight participants, each preflight participant will need to sign prior to the start of racing.
Club Waiver Form to be used at each tournament, one representative from each club will need to sign prior to the start of racing.
Preflight Entry Form to be used when entering a team for Preflight at a U-FLI event.
Singles Entry Form to be used when entering a team for Singles Racing at a U-FLI event.
Pairs Entry Form to be used when entering a team for Pairs Racing at a U-FLI event.
Standard or Variety Team Entry Form to be used when entering a team for Standard or Variety Racing at a U-FLI event.
Singles Time Sheet Time sheet that is used to score and track Singles Racing.
Pairs Time Sheet Time sheet that is used to score and track Pairs Racing.
Team Time Sheet Time sheet that is used to score and track Standard or Variety Racing.
Use of U-FLI Logo Policy This application is for those who wish to use the official U-FLI logo on promotional materials or other items.
Form What’s It Used For
Novice Junior Handler The first award in the Junior Handler Program. This form is to be filled out for the Novice level.
Intermediate Junior Handler The second award in the Junior Handler Program. This form is to be filled out for the Intermediate level.
Advanced Junior Handler The third award in the Junior Handler Program. This form is to be filled out for the Advanced level.
Form What’s It Used For
Height Sheet Used by the judge at each tournament for recording dog jump height measurements. This sheet should be made available during the tournament in a public area after measuring has occurred.
Participant Excusal Excusal form to be used by an U-FLI judge when excusing a competitor from an event.
Dog Excusal Excusal form to be used by an U-FLI judge when excusing a dog from an event.
Record Verification Used by head judge when verifying a new record for standard or variety teams.
